Maintenance and Retention of Records
We ensure
that your records are maintained for a minimum of 7 years or until age 21
in the case of minors. They
are stored in secured areas. It
is our mission to ensure the confidentiality of your records and
accessibility only to appropriately designated staff.
Release of
Information
To obtain
copies of your records or a minor child’s records: (click here for

In order to protect the confidentiality of your records, you must complete
a Patient Request for Records form. You
may complete this form in the Health Information
Management
department hours:
Monday through
Friday between 7:00 a.m. and 4:30 p.m.
You will need to present a picture I.D. at the time the request for
records is submitted.

To pick up
records for your spouse or other individual
or to have
someone else pick up your records:

We
must have permission in writing for you to pick up records or to have
someone else pick up
your records. You must have a completed
Authorization to Use or Disclose Protected
Health Information to pick up another individual’s records.
If someone else is to pick up your records, you must provide them
with a completed form to bring with them
to the hospital.

work with you to make other arrangements.
