Patient
Care Units
Medical/Surgical
Unit (MedSurg)
Care
is provided by and/or under the direction of Registered Nurses with the
assistance of Nursing Assistants
and Unit Clerks. This
unit is a combination of acute, swing bed and observation patients. Monitoring
and interventions are provided for patients at risk for altered skin
integrity, falls, minor cardiac and pneumonia. Multidisciplinary
teams comprised of Social Services/Discharge Planning, Dietary and Rehab
Services meet to discuss patient needs and plan of care. Patient
Care & Services
Nursing
All
nursing units use a planned, systematic and on-going monitoring and evaluation
approach to assess the quality of nursing care. Our
compassionate nurses and support personnel work with our physicians to provide
high quality, cost effective care using the latest in:
Discharge
Planning
For
most people, admission to, treatment and discharge from the hospital will be
successful and they will return to their usual way of life very quickly
through the provision of an accurate diagnosis, treatment and rehabilitative
service(s). Some
people will need additional help to enable them to do so. Identification of
these needs begins on admission in the form of discharge planning. Discharge
planning continues throughout the hospital stay as needs are identified and
care is planned to meet those needs. Case
Management and Social Services work with you, your family, your physician(s),
nurses and rehab staff to arrange the care needed following your hospital stay
including home care, nursing home care, rehabilitative care, other in-patient
or out-patient treatment and other help. Swing
Bed Program Hospitals
with a Swing Bed Program can use its beds to provide either acute care
(regular hospital care) or skilled nursing care level. Swing
beds are beds that can be used when patients no longer need acute care, but
still require skilled nursing care and rehabilitation. Sometimes
after an illness or injury has required you to stay in the hospital for an
extended period of time, you may reach a point where you no longer need acute
hospital care but are unable to physically take care of yourself at home.
Swing Bed care can be an option that does not require relocation after
hospitalization, or separation from support systems in the community. Our
nursing staff and Physical, Occupational and Speech Therapy Services work with
staff physicians to determine the best treatment plan for each patient, based
on the patient’s individualized needs. An Activities Director and Social
Worker are available as needed. Services
are provided until your condition is stable or rehabilitation potential is
reached and you are ready to go home or to another setting for additional
care. Some
examples of patient health situations that may be ideal for Swing bed an
option is:
